Step 1:

Let's start out by creating a new file. I used a 600x600 pixels canvas set at 72dpi, and I filled my background with a white color. Now create a layer and go to Filter > Render > 3D Transform. On the 3D Transform window choose the Cube Tool and draw a cube on the canvas window. Then choose the Trackball Tool and rotate the cube so each side has a flat shape or else it won't render.

Step 2:

Choose the Pen Tool and under the options palette select Paths. In a new layer trace the top part of your cube. Once you have a path shape on the top side of the cube, right click your mouse and choose Fill paths with black color shade.

Step 3:

Under Layer Style(Layer > Layer Style) add an Inner Shadow, Satin and Gradient Overlay blending options to the black skewed rectangle shape layer. Then set the layer's blending mode to Lighten.

Step 4:

In another new layer trace the left side of your cube and fill paths with black color shade. Now under Layer Style(Layer > Layer Style) add an Inner Shadow and Gradient Overlay blending options. Then set the layer's blending mode to Lighten.

Step 5:

Select your Horizontal Type Tool and above your screen under the options palette set the font family to Wingdings, 130pt, strong and black color shade. In a new text layer type '>' which is a biohazard sign.

Step 6:

Under Layer Style(Layer > Layer Style) add an Inner Shadow and Stroke blending options to your '>' text layer.

Step 7:

Now modify the biohazard sign design so it fits with the top side of the cube. Use the transform options such as skew, scale and rotate. Since your working with a text layer the option Distort or Perspective will not be available.

Step 8:

Here we added the designs for the other side of the cube.
